Safety Bulletin from MAIB, possibly applies to us

Thanks. I have not encountered that, but did once find that the tubular woven tape on the 'boat' end of a MOB system (presumably there as more resistant to sunlight) came apart from the main polypropylene line to which it was (inadequately) stitched.

It should be noted that when repacking the line after checking, that it should be stuffed into the bag, and never coiled.
I have noticed over the years that many throw lines found on boats are left in their bags since purchase and more often than not were coiled when packed by the manufacturer or were repacked as a coil subsequently. A throw line deployed in this condition will immediately tangle comprehensively and take some time to disentangle.

The proper use of this rescue aid should be demonstrated to crew and practised by all, so that it works effectively if it is ever needed.
