Not to the extent that I'd sign a document fraudulently claiming that I disinfect my lockers and then force someone who doesn't know I exist to receive the form.

I suspect the guy who deals with the form wouldn't have time or inclination to exact revenge but if he was caught on a quiet day in the wrong frame of mind I can guess which vessel he will be arranging the spot checks for!
