In view of the seriousness of this type of episode and jezjez's conclusions. Maybe, an article in the PBO may help to prevent a tragic accident in the future.

I am sure that very few people appreciate the way flue gas flow is influenced by various ventilation options. How many would have seen the danger looming when the tide turned? Or if the wind changed direction.
