Pet Passport - Non Boaty

We are just getting our dog ready for this ( we plan to drive to Spain next Winter ). Its had its rabies jab and blood test and just as we thought we understood the process, we have been told we need an export licence. Has anyone been through this whole process and can tell me what forms we need, i.e PETS1, PETS5 or what, and in what order do we collect these bits of paper? Cos I will be really in trouble with SHMBO if we get this wrong!
