Advice/opinions please..,
For an engine thru hull there are two types
Those that are a thru hull with a separate guard….and one piece thru hull and guard

Is one better than the other ?

Confused. Both pictures seem to show a single unit skin fitting with integral scoop/strainer, don't they?
Yes.....one online and the other in a shop. I thought that most people know what the separate ones look like

But the guards aren't just guards, they're scoops as well. To be mounted forward facing for an intake, and backward facing for an outlet.

To answer the original Bouba question, integrated scoops are obviously better from an engineering point of view, so should be fitted where you can, but if you want more access than those slots give you, for cleaning inside the skin fitting, then you must fit separate strainer guards.

Do remember though that Maestrini make the scoops with different slot widths. You can get a version with only 2 or 3 wide slots, rather than 6 or 7 narrow slots, to assist you in cleaning but still allow you to have a strainer of some sort.

That’s my point....the all in one unit are slightly more airstreamed....so could assist the water flow....but is it really noticeable?
But the separate grills are also scooped

I wouldn't lose any sleep on what you seem to be worried about.
The only thing that really matter with these things is that you should go for the most expensive you can find.
For instance, among the two posted in your OP, not only I don't know the brand of the first (which unless you know better is enough to rule it out), but it looks like brass (which you shouldn't touch with a bargepole).
The second is made by Guidi (which is good), but I can't be positive about the material from the pic alone.
In fact, Guidi manufactures either brass or bronze, to meet the demand of both penny pinchers and real boaters respectively! 😜
