Reminds me of ol' times' overloaded lorries crawling up hill at full throttle but at walking pace.

Too much fuel? Yes - much more than engine can convert to power being far from the correct rpm range
Caused by growth? Oh yes. A few well placed barnacles on my props reduced top speed by 6 knots (24 to18) - and it really was only a few barnacles.

I picked up a 1 tonne builder’s tote bag in the prop last year, any throttle resulted in black smoke and slightly singed diesel pouring out of the exhaust 😶‍🌫️

The boat got lifted out today.
I think forum wisdom is going to be proven correct.
It is clear that the owner does not believe it is the cause of his problems so hopefully he is in for nice surprise.

The small amount of antifouling that is visible in the first photograph looks very rough
Perhaps its time to strip the old antifoul and start again.

Seen lots worse that on a hull.
Unless the owner is super fussy and has time and money to burn, a good jet wash and nice new new coat of anti-foul will sort that hull.
Fouling on the sterngear is the real problem and more specifically the crud on the prop blades.

Once organised a cruise involving the club " speed merchant" with a Princess who moaned about the club cruise speed being set at 8 knots.
Far to slow he opined.
Halfway round to our destination all you could see of him was a plume of black smoke far behind in the distance as he struggled to keep up.
On arrival, a volunteer from the local diving club was summoned to access the problem, the hull was OK ish but the prop blades were smothered in barnycules and coral.
20 mins with paint scraper soon sorted that.
The amount of money the "speed merchant" offered the diver for his efforts was mean to say the least , of course, our fast friend left the rest of us to eat his "dust" on the way back.

Just got back from post scrape and paint sea trial.
What a difference.
WOT 3700
Top speed about just ovet 40kph.
One happy frenchman. (owner)
Very bow up. The flaps were not working ( now fixed.... blown fuze).
So should do better next time.
There still some black smoke. Could this be residue thst has built up in the engine burning off?
Thanks for your views..... which seem to have been spot on.

My 30DS is based on the broads so spends a lot of time pottering about at river speeds, the plume of crud on first open up is horrendous but soon clears, I think oil builds up in the turbos when they're not spooled up properly so build up could be some of the issue, as said try the italian tune approach and see what happens, mine has TMD41's.

Re continued black smoke that wasn’t there . If the Italian tune up does not clear it ?

Monitor the oil levels and report back .

Tough little cookies those 41 s …..but there is only certain amount of punishment labouring a diesel can take .
