Ipad Problem with French Préavis Form

I can't seem to get a copy of the above form onto my older ipad in a 'fillable' format. I've got it on what appears to be fixed PDF form but can't get it so I can fill anything in, which for a form that needs emailing is a bit of a handicap!
Ipad is running version 9.3.5 and won't take any higher. All the Adobe and other PDF editor apps I've tried aren't compatible or want version 10 or higher.

Is there a way through or am I in firmly the Apple $$$ Trap?

Do you have Notes?

If so, can you save the PDF in a new Note?

Here‘s an old PDF that I’ve found in Files, now saved in Notes.

You will need to be able to see the square with the arrow and the circle with the pen/pencil.
