It's the transom

Mmmm OK, this would indicate the transom possibly has a structural problem.

Take off the bracket and try to inspect the core through the holes you drilled by using a bit of bent wire (coat hanger, welding rod ?) to feel if the core is soft, just adding another outer piece will not stop the transom compression if internal problems.
