I had the same problem this season, dropped it, cleaned and lubricated it, and dried the threads and a tiny dot of supper glue on the very top thread on inner section tightened it up really quick, "Bingo", No, got gready used gas pliers and a screwdriver through the shackle hole which I managed to snap right off.
Cost me £108 for a new swivel unit from Sea Teach.
If the crancked shackle is fitted the wrong way round it will press against the swivel body and unscrew the top.

Hi again .... we spent ages on sorting out the foil ... the top halyard swivel never gave trouble before ... in fact I wasn't concerned about it ...
It used to have a "doughnut" diverter - but the guy who fitted it drilled it out so that it was a pain when raising / lowering genny ... it used to skew and jam on the foil. It was removed .. and that has probably been the result - now the halyard is not pushed away by the doughnut and catches the nut..... as you correctly summise.
I'm now trying to source a cranked link to fit ... thinking even a plate of suitable shape - doesn't have to be a shackle ... in fact with a plate ... I can shape the end that bolts to the swivel part so that it butts up to the swivel and cannot fold towards the nut.
I'm wary of loctite to the fitting ... as its glass filled nylon (GFN or similar) I would be worried about needing to break the loctite without breaking the whole fitting !!
Otherwise the repairs / service I did to the system has been a great success ... many thanks to you and others who assisted with info ... examples etc.

That would make it worse I think as then the halyard would lie more against the nut .... if not at least it would be similar and still likely a problem ....
Another has suggested ... Vice ... hammer ..... long shackle ... which is my thoughts ... but I also know that bending a shackle can weaken it considerably !! the amount of bend is severe.
If you want details of the bend I can measure ours.
Also bend it over a 3/4" / 1" drill, put a radius on the bend, probably what the orignal is, a bent long shackle.

I'm sorry, I forgot you sail with the headsails left over from the Cutty Sark, all that weight aloft may cause problems, you're right. No point in taking any risks eh? Please refer to step 3 above.
If I could find a suitable shackle out here to do the bend - I would ... but so far all I've been able to find out here is not suitable and when I spoke to the metals guy about it - he confirmed my worry that the shackle may actually get a crack or weakened area ...
I unfortunately since Russian restrictions on products exported to Baltic States can only get cheap chinese shackles here unless I order ... the cheapos being of metal not suited to cold bend.
