The drain hole on my anchor locker (16ft boat) is a small hole in the bow of the boat - Is this a normal arrangement? Would not water be pushed in when underway through a sea ? It's at the base of the locker, so presumably drains, but wondered if some sort of cover arrangement might be useful? width of the bow at the point of the hole is 10mm

Usual solution is one of these covers and a drain at the side but you don't have room for that.

You could try a rubber flap glued at the top and hanging down.

Seems like a nice, simple arrangement to me, with the benefit that it won't dribble rusty marks down the topsides.
Any arrangement pointing straight down will probably exacerbate ingress, as water would be forced in when the bow goes down - without aiding egress, and making cleaning more difficult. You'd need something that drains to the back and down - all terribly complicated to fix a problem that probably doesn't exist.

See post 2. Most drain holes are on one side or the other but seldom are designed very cleverly as most don't drain the locker completely. You could fill the existing hole, drill a new hole, to one side, and fit a clam shell cover.

I would not worry about it but leave as is.

However.... it depends on how much chain you carry (and what else you keep in the locker). You could add a false, perforated base to the locker - so just lift the base a couple of cm. This will allow the contents of the locker to drain and any seawater forced in will continue to drain and or sit below the contents. Anchor lockers are quite a complicated shape and fitting a false base neatly needs a lot of experimental bases before you get it right.

Again - I would not worry about it and leave as is.

I have seen much worse.

Do check, regularly, that the drain hole is not blocked. (and when you wash the decks - wash the locker of salt water).

Drain holes are usually above the bottom of the locker to delay getting blocked up from silt inside. Perhaps worth putting a bit of mesh grid inside to keep the rope/chain off the bottom.
