Things now tried:

Injector pulled and cleaned. No change.
Valve clearances reset. No change.
Poked around under the hp pump for the cold start mechanism as mentioned above. All seems in order. No change.

I think low compression is looking more and more likely. Doh! At least she fires up OK with easy start....!

My engine starts, but misfires and coughs like anything for maybe 20 seconds, with lots of unburnt diesel wafting from the exhaust. After that, sweet as anything, seems to have plenty of power. It’s still quite new to me so I’m working through the possibilities….
 
Mt 1GM10 had a starting problem as described after a full engine rebuild, we had lots of compression and the fuel system was bled several times. In the end I found that the engine fuel filter housing was leaking a tiny amount of air, new part fitted and problem solved!

Just run a temporary fuel line from the tank outlet to the liftpump on the engine.
Cheap clear plastic line will do.
Nothing to loose at this stage.
 
The fuel filter he is referring to I think is the one after the lift pump - bit more of a sod to bypass. I have bypassed the main filter and lift pump at one point just to test and it didn't help - but it could be an issue down the chain.

Hmm. I'll keep the updates coming as I get work done.
