Depends if it's registered and if, Part 1 or Part 3.

If not registered, then a form can be downloaded from the RYA website or supplied by a broker or, you can just make up a bill of sale yourselves.

If Part 3 (SSR) registered, the seller cancels registration and the buyer re-registers in his/her names. I'm not sure about Part 1, others will have to advise.

£25 for registering on SSR

£80 for change of ownership if Part 1 registered. Assuming new owner wants to maintain the registration on part 1
 
The Bill of Sale is the critical document as that is evidence of title. It should be accompanied by all the previous paperwork including previous Bills of Sale to show chain of title.

Part 1 registration is a register of title so documents have to confirm with requirements - details available from registrar. SSR is nothing to do with title but as Graham says selling owner should cancel and new owner re-register (if required and if new owner qualifies).
