Is anything like that actually needed. We visit the PP once a year for DEKPA stamp and payment and otherwise have guests for a few days or not as we sail about.

We used this exact form when we checked in at Symi in April, and have presented it in a few places along the way (I used Acrobat's Fill & Sign feature to fill it in). Everyone I have presented it to, has been happy with it.

Agree with both. Just got our dekpa. No questions or interest if we had any passenger's
