Shipping Forecast Template

Does anyone happen to have a template to record the shipping forecast, similar to those found in Almanacs? I have a copy of the shipping forecast map. Alternatively, does anyone know if there is an example on the internet ( I have searched without success ).

I have left my almanac on the boat and I want to laminate a copy or two for use on my next sailing expedition.
