I used to teach sailing fro a living so I've come across this myself many times.

The only real way to solve this is to get her to lay flat with her head the same height as her heart from the first moment she feels ill.

This almost always works.

She's tried travel sickness tablets that do help sometimes but I'm now wondering what others have found to work in similar circumstances.

It sounds counterintuitive but alcohol! Giver her a beer, or a glass of bubbly or some alcoholic ginger beer. If a glass or two doesn't work, then send her down below to sleep. When you close your eyes, you remove one of the senses.

I'll add some Sturgeon, Travel Calm, Ginger Nuts, Pepsi Cola and a bottle of dom perignon to our on board essentials list and just hope she doesn't take the lot in one go ;:nonchalance:

Scopoderm patches are excellent provided they don't make her hallucinate. They are only available on a private prescription and she should try one in the house on a day when she can afford to switch off for a few hours if she reacts badly. My wife used to get very seasick but I've seen her cooking a full English in heavy seas with one of those patches on.
