Penta jumping into gear

we've just experienced an instance of the port engine (Volvo Penta TAMD 122P) engaging the gearbox without a demand from the engine control lever. This is clearly quite concerning, what might have caused this to happen? electrical or hydraulic?
 
Was yours one that was affected by the wiring in the plug? It was a few years ago now but I had a boat with Volvo EDC engines recalled and a mod made, they were 70 something so smaller (can't remember exact model) does this sound familiar to anyone else? Have you tried a serial number check with your dealer to see if anything shows up?

That's was the 74/75 p series which required a wiring mod on the cables to the reverse gear solenoid.
