... and flooding the boat...

The threads just pop off... I have tried using PTFE tape, but no joy.

Seems like some sort of glue/sealant will be needed. Am thinking of trying CT1 - any thoughts from the panel?

I would cut that blue fabric sleeve back to the grey reinforcement then fit a standard plastic hull skin fitting in the hole in the flexible tank, with sealant, and fit the filling hose direct to that, without that black elbow.

Is the joint splitting under load (as the tank empties)? Examine the outer thread closely to see if there is a split. Sounds like something wore or split. Can you fit a jubilee clip to the outside of the threaded section? Does the join need supporting to stop it pulling apart? I have fixed similar problems with amalgamating tape and jubilee clips.

I had a similar problem which was only resolved when I got the tank part on a flat surface so the thread didn't distort. The hose connector then screws on correctly and seals without any sealant required

Good idea but... It would need to be at least a 2" fitting, and how do you get the inside flange through a hole smaller than itself?

Tried but I think the threads are permanently distorted. Have taken the tank out and got it flat, and it seems OK - but the tank surface is, of course, not flat when it is full.

You fit it in the same way that you fit an inflation valve into an inflatable dinghy. The flange goes through like putting a button in a button hole. The hole in the tank can be distorted into an oval so you can insert the flange of the skin fitting like a button. Try first with an scrap of non-stretch fabric with a 2" hole in it, take it to the chandlers and see if the flange of a 2" skin fitting will pass through.
