Had a similar incident in Crinan Basin a couple of years ago trying to park in the toilet corner. Got the barge wound up enough in reverse to actually steer and was smugly proud of my skill in manoeuvering her towards the wall, that is until I selected "forward" to bring her to a graceful stop. The throttle cable broke but my crew, waiting to take my lines, threw himself bodily 'tween stern and stone to prevent damage. I now have a stern fender for such attempts. Maybe the MSC Opera should consider more fenders?
 
P & O ferry, on the Dover/Calais run, a few years back, had a complete power outage close in the to the docking point. Not a hope of stopping her. One lady fell, i seem to remember, and broke a leg.

Bridge crew exonerated. I'm not sure if anybody was blamed.

With so much wiring/fly by wire type controls, I guess it has to happen occasionally.

One report I was listening to re the cruise liner, mentioned , briefly, a 'tug' problem but did not elaborate.

The most glorious episode of this nature was undoubtedly Everards' demolition of the public convenience whilst turning in Barking Creek some years ago.

The most spectacular might have been Cosco's spirited attack on the New Orleans Hilton with a Panamax bulk carrier back in 1996 - the ship had to stay put whilst the building was shored up - blackout at the wrong moment...
