Was unable to get full revs or anything like it in ahead yesterday. Opening the throttle further resulted in the engine threatening to die. White smoke, unburnt diesel on the water. All I had done previously was change the gearbox oil the day before, can't imagine that could be related.

Limped back to the mooring and tried the engine in astern - revved OK in astern pulling against the mooring, but lots of white smoke and diesel sheen on the water. Obviously, unable to test in ahead on the mooring.

Most likely scenario?

Seems you have a fuel problem. Four suggestions, in no particular order of merit:-
a) Check the fuel filters for blockage.
b) Check compression by trying to turn engine over with the starting handle. Two strong resistances per 2 revolutions.
c) Get injectors tested by local diesel specialist, (if available).
d) On fast tick-over, crack open each injector delivery union in turn, see if the engine will continue to run on the other, single, injector. If it doesn't you have a bad injector, or possibly fuel pump.
 
revved OK astern, but would not rev well going forwards? got a wetsuit?? :-)

sure it's prob not, but would be a quick fix, assuming you like swimming?
