If this is common practice, how do the do "Free acceleration" smoke assesment part of an MOT test ?

Its NOT common practice on motor vehicles, but on marine engines, probably is done using the idle switch in the throttle lever housing.

Let me take you up on your offer of thread drift - why is air charge cooler servicing so important?

Servicing charge air cooler depends on how cheap it is. I have a large bowmann cooler om my engine. Solid pipes. Plenty capasity. No problem. But I know about charge air coolers that needs service more frekvent. Fouling makes cooling capasity worse, pressure drop worse and some are leaking after a few years. A bad charge air cooler can kill your engine in several different ways.

A cooler leaking sea water is bad. You easily get corrosion in cylinders.On a parent bore engine like this, that means new engine block. Water is harder to compress than air. If you get a lot water you get hydraulick blocking killing your engine by overstressing it. In spesial conditions like we have where I live combination of cold seawater temperature and high humidity in air can make the air condensing in the cooler. That's dangerous as vel.

To high charge air temperature incrase temperatures. To high pressure drop decrease boost pressure making air fuel ratio worse giving high temperatures or black smoke.
