Ship collision in the Caribbean

Just saw this on G-Captain - a general cargo ship was hit on the starboard side of her aft accomodation block by a Geest reefer vessel off Haiti last Saturday.

Impressive photos of the damages - its amazing that nobody was injured.

Were they (both ships) not keeping a proper and effective look-out?
