Why not make a 6" thick seat? makes the plumbing easier, solves the problem, and allows you to make it nice and erganomic 
In that case you are very lucky. Current medical advice is that the knees should be raised several inches above the level of the upper thigh for a full evacuation. You can even get small steps to put your feet on.... The toilet sits flat on the floor and not on a raised step so looks like something out of a nursery school...

Perhaps because they are so easy to custom build and any plinth that Jabsco made would always be 'not quite the right height and shape?Cant believe that Jabsco dont sell a plinth as an option, there must be a market for it.
