The (English language) notes in the linked form indicate that:


"The form shall be transmitted by the harbour master’s office"

and that it is to be used where boats / skippers:

"exeptionnally arrive directly on the French soil outside of an official border crossing point".

from which I infer that:

One should send it to the harbour master / marina at one's French destination (for them to send on to the police)

and that:

It's not needed if you are entering France via an official port of entry.

Works ok on a PC but can be problematic on other platforms. You can download a few blank forms, fill them in with a good old pen and Email a photo.
or fill out the form as much as possible at home, print quite a few copies and fill out the relevant bits with pen when you arrive and hand to customs when you get your passport stamped.

In all the ports I know of you need to send the form in 24hrs before your arrival. The purpose of the preavis system is so that you can enter via ports that don't have a customs/douane presence.
