Just to close out the thread.

I have since run the engine without topping up for over 4 hours now and it has performed well. No water in the sump tray, plenty of hot water and engine temperature rock solid.

OTOH
You may be simply trying to keep the HE too full. Instead of topping it up fully watch it over a longer period to see if it finds a stable level somewhat below the bottom of the filler neck, but still just covering the HE tubes. If it does then all is well . Dont keep on topping right up .... there must be a bit of expansion space left in the top of the HE!

I have a pair of 4.270. The non turbocharged predecessor of the 4.236. I had exactly the same issue until I realised what VicS has pointed out. I was always topping the header up, then I realised that the stable level was about 2/3 up the tank. Basically I never now top them up and putting my index finger in the tank always reaches the level ( when cold and I never suck my finger afterwards!).

Cheers
