Bearing in mind the extremely low power of an outboard engine compared to a car engine (20hp) do people regularly change timing belts as per manufacturers recommendations, my handbook says inspect every 100 hours and replace as necessary?

seems a perfectly reasonable recommendation to me, i'd follow it. The power might be low, but the belt will be smaller too.

It does say check and replace if needed. Personally I think that's good advise

I watched two mechanics working changing the cam belts on the Maritme Police Rib in Bruces yard in Faro the other month. 250. hp each?
 
Timing belt on a lower power outboard? 100 hours or 5 years sounds about right. Also replace the bolt/washer as they a wear part too.
