One Filipino crew member died after a boiler in the main engine room exploded, causing the fire, the Sri Lankan Navy said. All other persons on board, 22 in all, were rescued from the ship, which is carrying 270,000 metric tons of crude oil.

Possibly lost the water in the boiler. Not much water in a water tube boiler so the safety devices are essential plus quick reflexes if they fail , especially the low water level systems as all that heat from the burners can do a lot of damage to water less tubes.
