Really sorry to hear of your loss and the circumstances that led up to it.

There is a lot more I would like to say (and I am sure others feel that way too), having read the MAIB report, but with a court case pending, its best to keep that until afterwards and not risk prejudicing proceedings in any way.

This capsize was merely the latest in series of "incidents" which occurred over past couple of years which Medway Ports totally failed to react to.
Had two prior serious sinking/capsize events been investigated and certain paperwork been properly checked for its veracity ,the most serious accident of all may just have been prevented.

You are, of course, dead right.

Safety regs are there to avoid obvious calamities, but some people just don't seem to grasp even tiny basics, like Kill cords and Lifejackets or Safety lines.

These are drummed into us by more experiences boat users from our first forays into boating, either from our club, or this forum, the RYA, ATYC etc...everywhere.

For commercial users to ignore this basic stuff is of course unforgivable.

Fred, if there is any good to come from this, it's the fact that everyone who reads reports like these are made aware, and hopefully good practises will prevail, and others lives will be saved. Cold consolation, but nevertheless consolation.

This whole thing has frankly chilled me to the bone

Fred, sorry for your loss.

Reading the report it mentions no paper charts, it's also worth noting as not mentioned in the report, the ancient model of Garmin plotter in the list of navigation equipment, is also not capable of running up to date charts.

Firstly, I would like to give my condolences for the loss of your brother. The report is shocking with a complete disregard for any form of safety and it seems unbelievable that the skipper is still able to operate a business with the catalogue of disasters before and after the fatal attempt to tow the Sirius M?

I also wanted to mention that it did state on page 21 of the report that additional to the Garmin GPSMAP 298, there was also a laptop running EURONAV seaPro. Although to attempt a passage like this relying solely on a laptop for navigation without accurate and up to date paper charts for back up is quite something. But the whole report is quite something.
