Any ideas why some fail and others don't?

Sandy or muddy shallow harbour can do it, coming and going with little water. Hayle in my case, always bumping along on the first and last of the tide

I had a dry exhaust, so the water outlet showed instantly any water shortage.

The great thing about having a FV deckwash is for a quick fix you can reverse flush the system, briefly. A bit messy.

BTW, a leaky intercooler pipestack can be repaired by putting a piece of studding through the leaky hole with rubber washers, if you can find which one. I was told they will stand three holes blocked, but it depends.

try taking out the insert on a D2-55 or 75 probably a 2 day job for a typical mechanically minded DIYer.
