Just had a skim through but it's interesting that the does not recommend double clipping any hoses for the reason he describes. Mine was obviously built to the spec he cites as I only have double clips on fuel lines but not on any water hoses.

When my boat was built, by Northshore mouldings, "double clamp all below waterline hoses" was an extra and cost the original purchaser £43 for all five.
 
I think double clipping is normal ... the problem in the pics was that the second clip wasn't being tightened up on the hard pipe the tube was being clipped to, thus trying to pull it off. I can't see a problem if both clips are tightened on the hard pipe underneath.

I can also recommend reading Ian Nicholson's book on the subject, and the one by (IIRC) Thomas Ask.

I tend to think if one clip is not good enough, get a better clip, not more of the same.

Agreed. Have there been a significant number of cases caused by single hose clips giving way? All mine are still double clipped, as the boat came, but in the cases when I have removed hoses it has been a real struggle to do so, even with both clips removed.

Des Sleightholme once referred in an Old Harry story to "A Westerly from Grimsby, all double-clipped hoses and radar reflector the right way up". I know just what he meant.

In that case I can't help wondering why it would cost $15,000 to replace the keel nuts. Perhaps there were other problems too.
