how does this cabin light come out ?

It probably is just held in place by metal spring clips, I'd get a knife and just gently lever it away from the base.
 
We have similar lights, but not the same - and now that I know how expensive they might be I will lock the doors more securely.

With ours the transparent, translucent part flips off, there is a little recess on the edge (at the interface of transparent and the base) that you prod a small screw drive in and twist. The housing is then screwed to 'whatever' - in our case the cabin roof or deck.

Except it cannot be the base as its the highest component ???

Ours (the assemblies that look like yours) were originally halogens, those long (15mm glass bulbs) with 2 pins at the bottom, that ignorants like me don't know the technical name for). We replaced them with LEDs secured on a flat wafer (maybe 20mm square), 4 LEDs per wafer. They are much thinner than the halogens and fit well. Frankly they are not as bright - but we have more powerful LEDs in reading lights as well.

Sorry but don't have pics - they never seemed very interesting :(
