Anyone know how to make a gooseneck (not the bird, the one used to prevent water getting up one's exhaust from the wrong end). New ones cost from £45 to £65 and are too flipping long. Must be easy to fabricate.

Any ideas? cut-away diagrams showing the innards?

On my boat the flexible exhaust is simply taken uphill to a rubber silencer just below the after deck. From there the exhaust pipe runs downhill to the outlet on the transom, which is a few inches above the water line. Perhaps I've been lucky but so far I have never had water come back into the engine, even with following seas breaking over the after deck.

I made mine from stainless steel pipe fittings the same size as the exhaust fitting. I used 3 elbows and streight pipe either screwed together or in my case I welded the fittings together. I also included a stainless ball valve so I can close the exhaust to prevent water ingress as my exhaust exits on my port side not through the transom as norm This also helps with security as it will prevent motor being started if closed.

What you have all said is basically my understanding also. I re-discovered a s/s box with inlet and outlet whcih appears to be a silencer. I had taken it out of service using a combined water trap and silencer. But I think I will bring it back into service as an addition to my current setup mounting it toward the rear at the transom.
Thanks for your input

Why not make one up with corrugated flexible exhaust hose from Vetus? This will easily make a 'U' bend, perhaps kept in a 'U' at the top with a couple of cable ties, and clipped to surrounding woodwork.
