Exploding Apple charger

Has anyone every heard of an Apple iPhone charger exploding ? Not a fake one but a genuine one from Apple plugged into a household circuit? I leave my charger plugged in on the boat but having heard of this I wondered if there was a safety issue here I was unaware of?
Apologies if there are other posts on this but was this a one off or something more serious?

There is a burnt out 80' yacht in Woburn Bay, Grenada, tied alongside a large steamer. Fire source believed to be an Iphone charger left charging a phone. Can be seen on google earth.

I've had many (genuine) Apple chargers (Macs and iPhones) plugged permanently in for many years now and none have ever done anything else than charge stuff. Can't say the same for the charger of my old battery powered drill, which almost set the carpet on fire (it was a Parkside from Lidl that had served me well for over a decade until the chargers flaming demise).
