I am in the process of getting my yacht MCA coded for chartering. I need an "Emergency Instruction Card for VHF operation" Is there a site where I can download one?
You will fiind the correct protocol (wording) in the RYA boklet on Radio operation so it is a simple job to type it out and print your own customised one then laminate it.

I received a self-adhesive, plasticised one which just had to have the boat's callsign/MMSC number and name inked in, when I registered for the CG66 service.
Cliff is right. Register for CG66 and you get sent a pack of useful info which I would certainly leave in the chart table for Charterers, including a 2/3 A4 size self adhesive Emergency VHF (inc DSC) which has spaces to insert your own Vessel Name, Call Sign, MMSI.
Free, which is a very good price.
The RNLI produce a booklet "Safety at Sea" which contains a procedure card.If you want an instruction sheet make up your own and laminate it you can have procedure on one side and how to use the radio on the other.I stick copies of procedure on inside of Log Book.front of almanac on pilot books and also one copy in heads so that seated person can read it whilst meditating.

I got the procedure cards at the MCA stand at LIBS
That the laid down proceedure is in the wrong order?
I offer the following scenario.
You are the duty CG officer and you receive the message as laid out on frankly's post.

Now lets assume that the battery is about to fail at any time!
Firstly, on hearing MAYDAY the duty officer knows that someone is in the crap and that the "vessel or crew are in danger".
Now what I would want to know is
1. Name of the vessel.
2. Where is the vessel.
3. How many people (crew) am I looking for.
I don't really need to know what the problems is (initially) - vessel sent out a MAYDAY so I know he's in the crap.
I don't need to know he requires assistance because he's already requested "immediate" assistance by saying MAYDAY.
My MAYDAY would always be
1. MAYDAY x 3
2. This is ..... giving vessels name x 3
3. MAYDAY ... vessels name x 1
4. My call sign is .... x 1
5. My position is .... x 1
6. I have x total persons aboard.
7. I am -sinking, on fire etc
8. I have the following ...... l/raft, PLB etc
9. I require immediate assistance - OVER.
BUT
If the boat was about to go down and I stood a good chance of the vhf failing at any time then I would resort to the following.
MAYDAY x 3
This is x 3
My position is x 1
I have total x persons aboard
I am sinking on fire etc and about to abandon etc.
That way I know that I have told the DO who I am, where I am and how many people to look for.
May be I'm wrong but it seems sensible to me to tell them where to look and for how many people to look for.
Thanks for your comment Peter.Just to let you know that I copied the procedure from the sheet produced by the "Sea Safety Liason Group"
Members of group are as follows.RNLI,MCA,RYA,BMIF,and a symbol mthat I can't read but may be Trinity House.
