Emergency Procedures sticker or layout?

My Emergency Procedures sticker is faded and tatty now, and I want to replace it.

It haas the VHF procedure for operating the DSC and the Mayday procedure information, with spaces for the ships details.

I cant seem to locate either a new sticker or a formatted version that I can print and laminate.

It was about A5 size

Anyone?

I would put the callsign as 'Mike Hotel Uniform Echo Two' just in case the radio operator temporarily forgets the phonetic alphabet.

Not been there or done that (touch wood) but thats how mine is set up as the RO is likely to be me and I forget things when I get over excited! /forums/images/graemlins/grin.gif
 
This fits an A4 page, suitable for my boat and radio and able to fit into a cheap frame from a well known store. The end result (I have altered it to leave blanks for the name, MMSI & callsign and converted to pdf) is available here:

Frame is screwed to bulkhead using keyplates. The top line has room for name mmsi & callsign in brackets.
