Mine does not look squared up.

Also to get card out I have to use tweezers.

The instructions with my example of this otherwise excellent bit of kit had the diagram of the card entering the slot the wrong way round. To insert the card you had to have it upside down compared to the diagram in the booklet. Annoying? Yes.
This may be relevant to your situation.

Good thing I never looked at the instructions before I put the chart card into mine, because mine worked first go!
