Slipping Anchor Windlass

I have a Simpson Lawrence Horizon 1500 Windlass. It has an Allen screw in the shaft for the gypsy which the manual says you slacken for manual hauling, however it regularly comes loose when using the windlass to haul up the anchor disengaging the gipsy from the motor.

It seems as though this screw is taking the torque of the motor? Do I have a part missing or some maladjustment?

It looks like the flats on the shaft are intended to take the torque and the screw is for retaining only.
I note that it has a left-hand thread which should make it self-tightening.
My inclination would be to use Loctite on the thread and just lift the chain off the gypsy for hand-hauling.
Be patient...an real expert will be along in a minute!
 
Perhaps item# 22 "friction cone" is worn?

If it is, would putting a washer between it and the head of the screw make a difference? I am assuming that the head of the socket screw presses the cone inwards.
