If adjustment of the gear selector doesn't de-shudder my engine I am going to have to bite the bullet and get at my cones. How horrible job is removing a 1GM10 gearbox in situ? I have a reasonable amount of room behind it but very cramped access over the top of the engine, though I job mind whipping the head off to allow a bit more room.

Yes, much easier to get it out in one go and work on the gearbox with the engine out. Disconnect everything, unscrewing the mounts from the beds. Rig your mainsheet on a 4*2 across the main hatch, shackle onto lifting eye, slide engine forward and lift. Can be done singlehanded if you are a martyr, but easy with a mate who follows instructions.
