Can anyone help as I am getting desparate????

I have a recurring problem with my gearbox, and I cannot find the cause only the effect. The issue is that after getting hot it becomes difficult to engage forward gear. When the box is cold, selection is perfect. When going into forwards there is a horrible noise which is the cone not quite biting. This is a safety issue as we are in the med. So when mooring stern to and wanting to stop the boat by engaging forwards, it does not reliably engage.

The history is as follows

2001 Yacht was built
Summer 2012 problem first started.
Winter 12/13 gearbox taken out and send back to Yanmar UK. Yanmar said better to have "reconditioned box" ie one that had been taken off an engine that was having a saildrive fitted, so essentially a new box. New box fitted

Spring 2013 - within 3 weeks of use, same problem started again. Lots of communication with Yanmar agents in Southampton who supplied the new box. Eventually found Yanmar gearbox man in Croatia. He took box out. He found a worn ring but thought that was not the cause, but rather the result of the incomplete engagement. He adjusted the setting whereby forwards is selected before throttle increases.

All well until this month. During which time we have been from Dubrovnik to Corfu and back and then a further months use this year

A few days ago the problem occurred again. Now back with same Yanmar man.

Causes that have been eliminated are:
The shaft turns freely, as I can turn it by hand
The selector is not the cause as the noise occurs even when forwards is engaged directly on the gearbox

Gearbox has now been removed again. Mechanic comments that he can fix any damage in the box itself but is at a loss to know why it has re-occurred and how to prevent it happening again

Has anyone had this happen to them and can tell me what to do?
