Whilst walking around my boat at low tide I noticed that the prop and shaft look fine from one side but that a shiny patch is showing from the other side. Is this a sign of a problem?

If my sterngear looked like that I would definitely be worried. That denuded patch is likely to severely affect the balance of the propshaft and could cause the P bracket to be shaken loose. Only solution I can think, although counterintuitive, is to try to remove an equal portion of the coating on the opposite side of the shaft. I would then gently abrade the 'shiny' bits of shaft to, hopefully, give some key for the new coating to take hold. Good luck.

I'm getting worried about these patches of bare material.
On the port side of the boat there is another which I am sure will affect the speed on starboard tack.

Seems that the barnacales are catching a deadly virus called Nostickumitus. This could be due to immersion in fresh water????

This is starting to look like an attack by a surveyor. These, not uncommon, creatures tend to choose a particular vessel then proceed to scrape small patches of coating from different points on the hull. Some say they are in search of moisture (but cosidering it's a boat they are nibbling at, why they need to burrow for moisture is a puzzle) others that they are looking for polyester mites (which I suppose is more likely). Whatever the reason for the scraping, you are left with the tricky job of making good. I would tend to go with Sailorman's solution and hope it self repairs, though it may take a while with that amount of damage.

Dear God, what am I doing?????

Oh sorry .. nah no problem ... just slap another coat of copperepoxygunge (c) straight on top .. fair it off a bit (on second thoughts don't bother) .. job's a goodun
