I have a Penta 2001 from 1986 that I have just finished
overhauling and was painting last night. In the process I
noticed an outlet with a hose fitting underneath where the
starting motor sits, about half way up the main body.
This was in a bit of a state and appears not to have been
used (or I havn't noticed it before) so I took it off and
opened it up (it has 2 parts) only to find an inner chamber
(pierced by 2 perpendicular holes) full of fine sand!
Up in the main body of the motor was a lot more sand so
I poked around with a screwdriver and out flushed a
lot of water.

Anyone know what this is?

It sounds like the water drain plug for the engine. The hose is not important - someone has attached it so as to catch the water. I drain my engine every year when the boat is hauled out so as to avoid frost damage and the hole is always blocked just like yours. It is designed so you can partially unscrew the inner piece to drain the water but because of the blockage I always have to remove it completely. I leave the outer part alone.
 
Thanks for the replies (and I posted the question on www.volpen.org.uk as well, by the way): it's a drain plug.

I realized this after cleaning up the plugs and finding that when screwed together again they formed a seal. I suppose you could back flush the cooling water jacket this way too if you wanted to de-scale or something.
