Passage Planning sheet

There used to be a downloadable passage planning form somewhere on the YBW site. I have been looking for it without success. Does anybody know a) is still on here and b) where is it? Ta ever so.

Sorry! WX is an old wireless operator abbreviation for weather. Morse was the original texting!
