Don't know if this is fits the criteria or not.

On the Australian news last night a local yacht builder was charged with four counts of manslaughter, after the boat he built capsized in heavy seas, after the keel snapped off.

The incident happened back in 2002, but now this precedence has been set legally, the accountability factor for builders has moved up a notch.

The keel, obviously alloy, had a straight-edge break about 1/2 a metre (yard) from the hull bottom. The racing yacht had a long narrow dagger like keel.
Seemed like a curious place to put a weld point, given the amount of stress that would be taking place.

I know yachts have lost keels before, but how accountable have the builders been after the event?
