If the engine note goes noisy and a steady stream of water comes out of the exhaust (tap left quarter on sort of thing), is that of concern? My thought is the exhaust has come loose or something.

Out of the exhaust into the sea? Or out of the exhaust into the boat?
If the former, the change of note suggest something inside the exhaust system. The OP doesn't describe the layout, but I'd imagine the water-trap (if fitted) would be favourite. If it's plastic/GRP, could the exhaust have run without water recently? If steel, perhaps something's corroded or become misplaced.
P.S. If there is a fault of this sort, it may mean the water trap no longer operates as a water trap. If there's a drain on it, it would be prudent to open it after stopping the engine. (And closing it, obviously, before running the engine again.)

Since the exhaust gas wasn’t pushing the water through, no pulsing ensued.
I'm surprised "a steady stream" of water came out of the exhaust at all. (Assuming you mean the rearward end where it's supposed to discharge.) Is there a swan-neck?
Wasn't the interior of the boat kind of smelly when the engine was running?
