A yachts person, Sarah Young has been lost but recovered dead from one of the Global Challenge boats. According to race organizers she was not clipped on at the time and doing routine tidying in the cockpit following reefing. This shows how critical clipping on is and even more so for single handers. Her body was recovered if that is any comfort for her family but that would be impossible for a single hander.

I shudder when I think of the times that I have not been clipped on in the cockpit in rough weather.
