I was going to suggest HMS Discovery, but only really covers points 1 and 3. Not sure shipping cargo to the White Russians counts as serving.
 
Name that ship

Take a brownie point Nigel1 - HMS Caroline was a dam fine answer even if not quite the right one. In fact the ship is berthed very near to HMS Caroline.
This is her berth nowadays - right back where she was built in fact.

The SS Nomadic was built in 1911 by H&W in Belfast and was a White Star line ship. She ferried passengers out to the RMS Titanic, served in both world wars as a troopship and has returned to the Port of Belfast, courtesy of the Nomadic Charitable Trust - what a wonderful little ship. Another good reason to call at Belfast Harbour Marina when next pootling along those shores!
