How the #&%¤# did this happen?

It is not uncommon when at or near sea at below freezingpoint.

Spraying seawater and/or precipitation at certain metheoroligical circumstances (like underchilled rain or snow falling through higher colder layers of air) may cause it.

Have you seen Deadliest Catch.

Those poor sods work like dogs for many hours straight then have to smash the ice off 28m+ boats before getting any sleep.

In the current series there was one boat that was putting on over an inch of ice an hour due to the freezing spray. I dread to think how much weight that is but got to be in the tons per hour.
 
I remember arriving in South georgia and having to get up early to chip the ice from the upper deck - looked something like the photos and there was some concern about stability (Type 42 Destroyer). Quite an experience and some good photos.

The skipper and crew have been travelling around this country recently like celebrities, addressing sell-out shows on what sounds like random banter based on audience questions.
