I get a diver to clean the sterngear on my boat about 3 or 4 times a year and the hull is re-antifouled once a year. I find that fouled props make the biggest difference to speed. In the Med it's quite common to lose 5 or 6 knots in only a few weeks
Yes, I'd echo Mike's comment. Our's is in the marina a far bit, SWMBO only likes flat water, so I end up doing the same
"Bob the diver" 3 times a season.
Theoretically, annually ...... but then it depends how busy I am. Quick scrape and leave the antifoul where it was this year - except for outdrive, props and trim tabs which got done.
Suspect I'll pay next year .... but then as I'm giving up work in January - I'll have the time to do it properly.

Agree, I'm doing an experiment at the moment. Last month in Spain I smeared Lanoline all over the props, while they were underwater, before leaving the boat (I can just about do this on my S37 hanging over the bathing platform with the props raised) I return in a couple of weeks ...lets see if it worked??
Depends how bad the fouling gets with us. Paint applied either 1 or 2 yearly.
Have the advantage of a drying mooring so can regularly clean hull with a garden hoe.
Outdrive and prop seem to get more fouled than hull and are far more of a nuisance to clean. Have yet to discover an antifouling that really works with them.
