Handy form for writing down weather info?

I'm sure I've seen this referred to here but now can't find it. Useful proforma for getting the weather info down in an organised way? Ring any bells? Ta.
 
On ships as Second Mate - I used to make up a hard card with clear plastic stuck over it .... then a chinagraph pencil could mark it and be wiped off .... It used to have the wx areas listed on it and boxes to mark in against each one ... double-sided to get them all on of course .....

Then later I made up ones with card and covered in chart paper - its strong and erasable with standard 2b pencil.

I enlarged the card system to tides, currents, VHF Mayday, sextant sights .... all sorts .... made one for each ... I found that many guys I sailed with adapted the idea to suit themselves etc.

I always found that "commercial" or printed styles had that little bit missing or needed a tweak somewhere ....
 
I've got similar cards on my boat, but I've laminated them with non-reflective matt finish laminating pouches. You can write in pencil on them. The pencil won't rub off and is waterproof. Clean it off with Cif or similar.
