Thanks Nigel, I'd forgotten that I had a copy of this. I think that I picked it up in 2004 at the first LBS in Excel or perhaps the following year.

My copy is Revision 8, July 2004 and is a double sided blue book with "The Book" and "The Products" combined (just turn over to get other document).

I've downloaded Revision 9, June 2011 and will see how it compares with my old paper copy.
