We are talking about a Bridge Full of Officers here all with Navigation experience not some Civvies learning how to plot a Course from Calshot Spit to Cowes.
The proper method on ANY Bridge is whom-so-ever has done the plot has it checked several times by others. THEN even the Cadets have a looksee. (In the Navy's case that would be whatever Snotty is on watch.)

I can think of a few other major cockups in recent years.

Nottingham

Endurance

Both nearly sunk due to incompetence and pizz poor training.

The report into the near sinking of the Endurance is due out very shortly.
I cant wait to see if it lays blame on anyone. Especially a certain Rating (female)
